Minor tattoo
Florida requires written notarized parental or legal guardian consent for tattooing minors age 16 through 17, along with the other identification and guardian requirements provided by state law.

Complete the Ink Ink client record digitally before your procedure, or use the printer-friendly 8.5 × 11 version.
Minor piercing
Florida requires written notarized parental or legal guardian consent for body piercing of a minor. A parent or legal guardian must also be present for a piercing when the minor is under age 16.
